When Field Evolution was founded, owner Morgan Bemko knew a digital form solution was essential for completing customer job sheets in the field. “We started with GoFormz because when I created Field Evolution, one of my business partners was already using it,” Bemko explained. However, they still evaluated other digital form providers before making a final decision.
“We looked at doForms and FastField Forms because of one of my business partners, but what set GoFormz apart for us was the live feed of forms populating in the Forms tab,” said Bemko. Within GoFormz, users can view both incomplete and completed forms in real time, ensuring seamless access to data from any location. This became a critical feature for the Field Evolution team. “I can even tell when a guy is in the middle of a job because I can see the form saved as a draft rather than marked as completed in the Forms tab. I think GoFormz is the only company that offers this—other platforms require you to hit ‘complete’ before the information becomes visible.”

We can then tell the customer, ‘The installer is completing installations every thirty minutes.’ It allows us to track work being done on-site, even from a remote location.”

Field Evolution relies on GoFormz to complete customer-specific digital job sheets. “We made this form as simple as possible to understand. You can see clearly the camera information, what the power connector looks like, a picture of the VIN number, a picture of the installer using the application, and then the location of the installation, so the customer can’t argue that we weren’t there,” explained Bemko. Field Evolution’s job sheets rely heavily on Image Fields to visually document their work, capturing before-and-after photos as proof of completion and a safeguard for their business. Additionally, Location Fields are used to verify where the work was performed and confirm the installer’s location at the time of installation.
With auto-email notifications, digital job sheets are instantly routed to their designated recipients upon completion. “Our end customer gets a copy, the installer gets a copy, and we have the live feed along with all the data stored in GoFormz for us to reference,” explained Bemko.

Job sheets are essential to Field Evolution’s operations. “We have job sheets because we’re installing physical equipment into vehicles, and they serve as a critical part of our warranty process,” explained Bemko. “I’ve seen cars burn to the ground, and we’ve been blamed simply because we were the last to work on them. We call those thermal events, and I’ve handled hundreds of them in court cases. That’s why job sheets are so important—they protect our business.”
Image fields are also used to safeguard the business. “Pictures are crucial because, as I mentioned, it's a low-return, high-risk situation when customers call with damage claims, saying we broke their dashboard or wired something incorrectly. With the way our forms are set up, all I need is a truck number, and I can immediately pull up the job sheet for that vehicle,” explained Bemko. “We can see exactly what was done. So, when a customer questions the work, we can show them the proof. At the end of the day, it’s all about protecting ourselves.” This has saved Field Evolution thousands of dollars. “GoFormz has easily saved us hundreds of thousands of dollars in damage claims.”

Field Evolution manages a nationwide network of 125 subcontractors, many of whom are direct competitors. To maintain confidentiality and streamline operations, the team utilizes User Group permissions in GoFormz, ensuring each subcontractor only has access to the forms and template folders relevant to their business.
“Our 100+ installer base is made up of about forty different companies. A feature that’s been important to how we operate is the User Group permissions in GoFormz,” explained Bemko. “We don’t want someone from Company A seeing the work of Company B and wondering, ‘Why did they get that job over us?’ Since they’re all technically competitors, we’ve organized each subcontractor into their own Company Group with dedicated Company Job Folders. This way, everyone only sees what pertains to their work.”
Field Evolution uses GoFormz as a selling point of their services, demonstrating its use to potential clients and bringing in new business. “I sell our service by demonstrating to new people what GoFormz does. This software allows us to proactively monitor installs regardless of our location, so I don’t have to be on site–I can just watch the updates come in through GoFormz.”
Field Evolution has realized significant savings across various aspects of its business, including reducing the need for follow-up work. “By eliminating follow-up work, we’ve effectively saved the cost of two full-time positions each year, which amounts to approximately $80,000–$90,000 in annual savings,” explained Bemko.
